Doc-99 Chemist™ is the optimization of medicine service that integrates with online pharmacist advice and off-line service to customers to purchases of supplementation products and body healthcare checks. The Doc-99 Chemist™ lets individual and company user access his or her or member of a company plan for healthcare as incentive to a mobile care plan, or on the go. With the Doc-99 Chemist™, you can access your healthcare online with an appointed pharmacist advice and off-line to go pharmacies shopping anytime and anywhere! Features of Doc-99 Chemist™

• Access and share all your healthcare measurement to keep healthy include blood pressure, oximetry, body fat and hydration, blood cholesterol, blood glucose, uric acid and BMI.

• View your present healthcare check measurement on a Health House in a pharmacy,

• Watch previous healthcare checks in designated formats showing acceptable range high, low and measurement, • Auto loading your body healthcare measurement to the smartphone

• Using “Care Plan” items to manage your healthcare via designated web portal such as (www.smart-health.cn) You can choose different yearly plan to suit your needs! Healthy Men Healthy Women Child Chronic illness Palliative Doc-99 Chemist™ Management Platform When you complete the registration process, you can log in Doc-99 Chemist™ portal www.doc99.hk to manage your healthcare.

Terms and Conditions

Doc-99 Chemist™ Service Subscription terms

1. I/We agree to enter into this Agreement with Abenefits Sourcing Limited (ABS) for the provision of the Doc-99 service by ABS subject to the attached terms and conditions entitled “Doc-99 Chemist™ Service – Terms of Use”.

2. I/We will pay the following fees of the selected option to Doc-99 Chemist™
i. A fee for each year of this Agreement
ii. HKD990 for supplementation coupon worth HKD1000 or
iii. HKD990 for Care Plan or
iv. HKD990 for Health House e-health measurement

3. I/We will be liable for the entire fee if this Agreement is terminated prior to the expiry of the corresponding yearly period.

4. If I/We terminate this Agreement or cease to be a customer of ABS for mobile healthcare service; all information associated with my/our account of the Doc-99 Chemist™ service will be deleted after such termination of this Agreement or cessation of being a customer.

5. If I/we cease to be a customer of ABS for mobile healthcare service, this Agreement shall be deemed to be terminated by me/us upon such cessation of being a Doc-99 Chemist™ customer.

6. For the purpose of the attached terms and conditions entitled “Doc-99 Chemist™– Terms of Use”, ABS is referred to as the Authorized Distributor.

7. I/We understand that the signing date of this Agreement will be the commencement date of this Agreement between Bitcasa and me/us, and that the yearly fee will be incurred from such commencement date.

All care plans are required to enter into the specified contract.

Doc-99 Chemist™ client is required to pay $ 990 annual fee. Doc-99 Chemist™ Upon purchase, the related costs are not refundable.

All services shall comply with Doc-99 Chemist™ service usage policy and fair use policy.

Unless otherwise noted, all service charges and fees are only suitable for local services.

All Doc-99 Chemist™ services and charges are published as final. Subject to change without notice.
